Providence Healthcare & Rehab Center, located in Palos Heights, Illinois is seeking a full time Restorative Nurse to join its clinical team. The primary purpose of your job is to assist the Nursing Department in planning, organizing, evaluating, and directing the overall delivery of clinical services and restorative programs governed by the State of Illinois. Knowledge of the MDS is also a crucial part of this position, specifically when conducting resident assessments and completing restorative care plans.


Summary of essential job functions

  • Be responsible for the writing and creating of all restorative programs for LTC and Medicare residents. Assist with the direction, participation, and evaluation of the restorative program.
  • Be responsible for the completion of Section G on the MDS, and the CAA’s and Care Plans.
  • Attend Medicare, fall meeting, QA meetings, and various care plan meetings as they arise.
  • Make daily rounds to evaluate resident care, the progress of individual residents, monitor restorative practices, and assist staff with restorative nursing care and educational needs.
  • Be responsible for ensuring that all responsible staff are educated and completing restorative nursing documentation in the EMR, to include but not limited to: ADL daily documentation, ADL (7) day look back tracking forms, monthly and quarterly restorative progress notes, endurance assessments for any resident with more than two restorative programs.
  • Be responsible for coordinating all restorative programming and documentation.
  • Perform or supervise the performance of restorative care.
  • Assess all LTC residents for: Need for wheelchairs or other assistive devices, Appropriate fit of wheelchair and other assistive devices.
  • Provide for appropriate positioning devices to ensure that residents comfort, position, and safety needs are met.
  • Provide safety/personal alarms for residents per protocol and ensure that: Alarms are in place and in working condition, CNAs check alarms every shift and replace as necessary, A list is maintained of all residents with alarms, investigate all falls and put new safety intervention in place. 
  • Maintain a list of all high fall risk residents and provide to IDT.
  • Monitor dining rooms to ensure that any residents with feeding difficulties are supplied with proper assistive devices.
  • Must be able to push, pull, move and/or lift a minimum of 50 pounds to a minimum height of 3 feet.


Skills necessary

  • Registered Nurse and/or LPN in the State of IL with active, unencumbered license.
  • Previous experience in long term and post-acute care for 3-5 years.
  • MDS 3.0 certification preferred.
  • Must have extensive working knowledge of RUGS.
  • RAC-CT certification strongly preferred.
  • Must be familiar with developing care plans.
  • Must have working knowledge of the restorative programs in long term care governed by the state of IL.
