Physical Therapist (PT)

Gary, Indiana, United States Part-time

Providence Life Services is seeking a Part Time, Registered PT for the Home Health division for our Indiana location primarily in the Gary and East Chicago area. 

Summary of Essential Job Functions:

  • Perform an initial evaluation on each client, which includes a medical history and other relevant information in order to provide appropriate treatment.
  • Assesses functional levels of clients and identifies problems/needs that require therapy intervention in order to provide appropriate treatment.
  • Collaborates with the physician, client, and SN on the Plan of Treatment, which includes client problem/needs, treatments, goals, and discharge planning in order to provide coordinated care.
  • Observes, records, and reports problems, progress and changes to the physician and other team members in a complete, accurate, and timely manner in order to provide quality care and maintain continuity of care.
  • Re-evaluates and revises the Plan of Treatment every 30 days and as needed when further services are necessary in order to provide appropriate therapy services and comply with regulation.
  • Communicates with the Director of Rehab regarding initial evaluations, discharge plans, weekly schedules, and absences in order to provide continuity of care and adequate coverage for clients and caseloads.
  • Supervises and instructs Physical Therapy Assistants and documents supervision per agency policy and procedures in order to comply with standards, regulations, and maintain high quality of care.
  • Develops and instructs the client, family, or caregiver on the home programs in order to improve functional levels, activities of daily living, and maximize the client’s potential.
  • Complies with Universal Precautions and Infection Control Policies in order to provide for a safe environment for the clients, families, and self

Qualifications :

  • Must be a graduate of an accredited physical therapy program
  • Must be licensed in the State of Indiana
  • Must be CPR certified.
  • Must have a minimum of 1 year Home Health exp. 
  • Must be able to push, pull, move and/or lift a minimum of 50 pounds to a minimum height of 3 feet.
