Social Worker/Case Manager


Providence Hospice offers services to clients and their families at end of life, whether that is the last day, week, month or year.  As a social worker you will have the privilage of caring for these families in their homes, nursing homes and assisted livings.  



Northwest Indiana



The primary purpose of your job position is to evaluate client and family needs and support them through the end stages of life and to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.             

Summary of essential job functions

  • Emotional support of families and clients
  • DME ordering and planning discharges from hospitals and/or nursing homes
  • Visiting patients and families on a weekly, bi-monthly, or monthly basis to provide ongoing counseling and support as patients are transitioning and dealing with end of life issues.  

Skills necessary

  • LSW required in the State of Indiana and Illinois
  • MSW required in the State of Indiana and Illinois
  • Miniimum of 1 year Hospice experience required
