Providence at Home is currently seeking full-time Hospice C.N.A’s for day shift to service our Homer Glen, Crete and Palos Heights Facilitiles as well as in home care in the surrounding suburbs. This is an excellent career opportunity for individuals who are truly passionate about providing care for the elderly!

 

Description

The primary purpose of your job position is to provide each of your assigned clients with personal care and services in accordance with the client’s home health aide plan of care as developed by the nurse case manager. Typical hours are Monday - Friday from 8:00 am - 4:30 pm.

 

Summary of essential job functions

  • Maintains intake and output records as instructed.
  • Checks and reports bowel movements and character of stools as instructed.
  • Assists with lifting, turning, moving, positioning, and transporting clients into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
  • Assists clients to walk with or without self-help devices as instructed.
  • Measures and records temperatures, pulse, and respiration’s (TPRs), and blood pressures as instructed.
  • Checks each client routinely to ensure that his/her personal care needs are being met in accordance with his/her wishes.
  • Observes and reports the presence of pressure areas and skin breakdowns to prevent decubitus ulcers (bedsores).
  • Provides indwelling catheter care.
  • Reviews care plans to determine if changes in the client’s daily care routine have been made on the care plan.
  • Informs the Case Manager, of any changes in the client’s condition so that appropriate information can be entered on the clients care plan.
  • Ensures that notes reflect that the care plan is being followed.
  • Provides Range of Motion Exercises as instructed. Record data as instructed.
  • Observes disoriented and comatose clients. Records and reports data as instructed.
  • Assists clients with dental and mouth care (i.e., brushing teeth/dentures, oral hygiene, special mouth care, etc.).
  • Assists clients with bath functions (i.e., bed bath, tub or shower bath, etc.) as directed.
  • Assists clients with hair care functions (i.e., combing, brushing, shampooing, etc.).
  • Assists clients with dressing/undressing as necessary.
  • Keeps clients dry (i.e., change clothing, linen, etc., when it becomes wet or soiled).
  • Changes bed linens. Keep linens tight to avoid wrinkles from forming under the client.
  • Positions bedridden clients in correct and comfortable position.
  • Assists clients with bowel and bladder functions (i.e., takes to the bathroom, offer bedpan/urinal, portable commode, etc.).

Skills Necessary

  • Must possess, as a minimum, a high school graduate diploma.
  • Must be C.N.A Certified in the State of Illinois with 5+ years CNA experience.
  • Previous Hospice experience
  • Reliable transportation
  • Must have a compassionate, empathetic and nurturing personality.
  • Must be able to push, pull, move and/or lift a minimum of 50 pounds to a minimum height of 3 feet.
