Providence Life Services offers a full range of services for families who need retirement living, assisted living, rehab, skilled nursing, and more. Emerald Meadows, our Assisted Living and Memory Care facility located in Grand Rapids, Michigan is seeking Part-time LPN's FOR ALL SHIFTS to join their clinical team in delivering quality care to its residents in efforts to improve overall quality of life.

The primary purpose of your position is to work along side the care team and supervise the healthcare activities of the facility during your tour of duty.  Such supervision must be in accordance with current federal, state, and local standards, guidelines, and regulations that govern our facility, and as may be required to ensure that the highest degree of quality care is maintained at all times.


Summary of essential job functions

  • Assist the Director of Clinical Services in directing the day-to-day functions of the healthcare activities in accordance with current rules, regulations, and guidelines that govern the long-term care facility.
  • Ensure that all healthcare service personnel comply with the procedures set forth in the policies and procedures
  • Assist in developing methods for coordinating healthcare services with other resident services to ensure the continuity of the residents’ total regimen of care.
  • Ensure that all healthcare service personnel are in compliance with their respective job descriptions.
  • Meet with the personnel, in planning the department’s services, programs, and activities.
  • Interpret the department’s policies and procedures to personnel, residents, visitors, and government agencies as required.
  • Assist with scheduling and management of call ins.
  • Develop work assignments, and assist in completing and performing such tasks to include providing care or medication administration to residents.
  • Ensure that all trainees are under direct supervision.
  • Make daily rounds of the department to ensure that all healthcare service personnel are performing their work assignments in accordance with acceptable healthcare standards.  Report findings to the Director.
  • Notify the resident’s attending physician and next-of-kin when there is a change in the resident’s condition.
  • Review documentation to ensure that it is informative and descriptive of the healthcare being provided; that it reflects the resident’s response to the care, and that such care is provided in accordance with the resident’s wishes.
  • Monitor medication passes and treatment schedules to ensure that medications are being administered as ordered and that treatments are provided as scheduled.
  • Monitor healthcare procedures to ensure that supplies are used in an efficient manner to avoid waste.
  • Ensure that residents who are unable to call for help are checked frequently.
  • Attend and participate in continuing educational programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status.
  • Must be able to push, pull, move and/or lift a minimum of 50 pounds to a minimum height of 3 feet

Skills necessary

  • Must be licensed as an LPN in the State of Michigan without any sanctions.
  • CPR Certified.
  • Must have experience in a hospital, long-term care facility, or other related health care facility
  • Compassionate and empathetic personality.
  • Must work every other weekend and holidays.
