Since 1993, Providence at Home has been serving the communities in the Chicago Metropolitan and Northwest Indiana area providing Homecare, Hospice and Private Duty services.  Providence at Home is a growing division of Providence Life Services which offers Independent Living, Assisted Living, Memory Support, Rehab Services and Skilled Nursing Care in our senior communities in 3 states.

The Quality Assurance Coordinator will report directly to the Vice President of Home and Community Based Services and will lead the quality operations of our Home Health and Hospice Agency. This position will be responsible for:

  • Responsible for the implementation and monitoring of the organization’s quality improvement activities and programs for home health, hospice and private duty services
  • Maintains knowledge of local, state and federal regulatory requirements and guidelines, inclusive of, but not limited to Joint Commission, CMS/Medicare, OSHA, CDC, and OIG guidelines
  • Provide a process to ensure employee orientation, competency and in-service education programs. Assists in the identification of skills for annual competence and provides in-service training support.
  • Acts as a resource to managers and staff regarding standards, policies and regulations
  • Maintains current knowledge related to Medicare Conditions of Participation and the Joint Commission’s standards to ensure that the organization and policies are compliant with state, federal and accreditation guidelines
  • Maintains current knowledge related to home care safety and infection control standards
  • Reviews all incident reports for completeness, risk, standard of care determination and appropriate follow-up
  • Monitors patient satisfaction outcomes and collaborates with management staff in follow-up of concerns expressed by patients, families, and physicians
  • Works collaboratively in order to assure documentation is complete and consistent with care and reflects legal/regulatory requirements
  • Compiles, trends and reports quality data in the following areas: patient care, safety, risk management, infection control, outcomes, and customer satisfaction. Data is analyzed and compared internally and externally with other sources when available
  • Coordinates activities related to clinical record review for data collection. Oversees peer review and all audit activities of the organization
  • Ensures that there are processes in place to monitor and measure all activities related to quality improvement. Provides direction and coordination of quality improvement activities for home health and quality assurance performance improvement (QAPI) activities for hospice, utilizing continuous quality improvement principles and methodologies. Chairs the Quality Improvement Committee

Position Requirements

  • RN licensed in the state of Illinois and Indiana
  • Minimum 2 years of experience in Medicare-certified skilled home health care and/or hospice agency
  • Minimum 1-2 years of experience in a quality improvement role and with Joint Commission accreditation
  • Must understand home health and hospice regulatory standards – both state and other accrediting agencies
