Park Place of Elmhurst, a CCRC Community offers a full range of services for families who need retirement living, assisted living, rehab, skilled nursing, and more We are seeking a Full- TIme Social Services Director to join its clinical team in delivering quality care to its residents in efforts to improve overall quality of life.

 

Description

The primary purpose of your job position is to plan, organize, develop, and direct the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, our established policies and procedures, and as may be directed by the Administrator, to assure that the medically related emotional and social needs of the resident are met/maintained on an individual basis.   As a resident advocate, the Social Service Director has direct contact with families, problem solves and participates in meetings with program leadership.  

 

Summary of essential job functions

  • Plan, develop, organize, implement, evaluate, and direct the social service programs of this facility.
  • Assist in the developing, administering, and coordinating of department policies and procedures.
  • Review departmental complaints and grievances from personnel and make written reports to the Administrator of action(s) taken. Follow facility’s established procedures.
  • Develop, implement, and maintain an ongoing quality assurance program for the social services department.
  • Work with emotional problems including assisting resident/family with anxieties and stress caused by illness and admission to the facility, difficulties in coping with residual physical disabilities, fears related to helplessness and death, and the need for institutional and specialized care.
  • Assist in interpreting social, psychological, and emotional needs of the resident/family to the medical staff, attending physician, and other resident care team members.
  • Provide information to resident/ families as to Medicare/Medicaid, and other financial assistance programs available to the resident.
  • Assure that social service progress notes are informative and descriptive of the services provided and of the resident’s response to the service.

 

Skills necessary

  • BSW or related degree and experience in long term healthcare/skilled nursing.
  • MDS knowledge preferred.
  • Responsibilities include, but are not limited to, managing all social services, documentation, computer work and discharge planning associated with the position.
  • Compassionate and empathetic personality.
